FAVN Report Instructions
You are responsible for entering the information on the FAVN REPORT FORM, with the knowledge that it will be used by government officials. A complete FAVN REPORT FORM is required for each sample submitted and the original form is retained on file at the Rabies Laboratory. Results are reported in International Units per milliliter (IU/mL) and a titer of greater or equal to 0.50 IU/mL is required for pet travel to rabies-free areas. The rabies antibody titer results are reflected on an adhesive sticker which is applied to a copy of the FAVN REPORT FORM – this is the official pet travel document.
Complete the FAVN REPORT FORM on-line and print if possible. Otherwise, handwritten information must be printed clearly in black ink.
Record the destination the animal will be traveling to. The destination is for Rabies Laboratory use to determine where to send the official FAVN REPORT FORM (stickered). Some destinations require us to send the report directly to them. If the travel destination changes (example: travel to Japan instead of Hawaii) after the test is finalized, it will not affect the validity of the FAVN results, however we cannot amend the FAVN REPORT FORM to reflect a different destination.
Record the submitting clinic contact information as we will mail the official FAVN REPORT FORM (stickered) to the submitting clinic – unless as stated above, we are required to send to a destination. Refer to “SENDOUT OF FAVN REPORT” below for additional information.
Record the owner’s first and last name and the animal’s name. The microchip number must be recorded on the FAVN REPORT FORM and the serum tube. Microchip identification must be implanted prior to sample draw. If an animal has two microchips, only the first number listed on the FAVN REPORT FORM will be provided on the result label. The Serum Draw Date must be completed by mm/dd/yyyy. A waiting period of 21-30 days between rabies vaccination and blood sampling is advised for optimum antibody response. Always verify requirements with the destination country as some have a longer wait period before the serum can be drawn.
The Species/Breed, Sex and Age and Rabies Vaccination History are all for the Rabies Laboratory use only. Include up to 3 vaccination dates if available, beginning with the most recent shot administered.
The veterinarian must sign the completed submission form, thereby confirming all information on the FAVN REPORT FORM is correct. If there are questions concerning the submission, the Rabies Laboratory communicates directly with the submitting clinic by fax, email or a phone call. Please be sure and record this information on the FAVN REPORT FORM.
Due to stringent regulations, personnel at the Rabies Laboratory are not permitted to add or change information on the FAVN REPORT FORM. Once results are finalized, no changes to the information on the form can be made by the submitting clinic. If an error is detected on the result label after the test is finalized - and our laboratory confirms a handwriting misinterpretation - the submitting clinic will be required to provide a letter explaining the error and pay for a reissue of the FAVN REPORT FORM.
1 -2 mL of serum with no anticoagulants or additives. Spin and separate serum from clot and pour into a 5.0 mL test tube. If quantity is not sufficient, due to leakage, damage or original volume, we will notify the veterinarian.
The serum sample must be relatively clear because grossly lipemic or hemolyzed samples are not viable for testing.
The tube must be labeled legibly with the animal’s microchip number even if it is the only tube in the package. The label must be securely attached to the tube so that it cannot be separated, erased or become illegible in transit. We cannot accept tubes that are not labeled.
Please refer to the current turnaround time on our website as it is subject to change. If you need results in less than 3 weeks, contact our laboratory before submitting your sample.
Place in the Package:
- The sample tube inserted into a plastic bag with enough absorbent material to prevent leakage. More than one sample from different animals can be sent in the same package.
- The FAVN REPORT FORM and any accompanying paperwork inserted into a plastic bag for protection against moisture.
- Pre-frozen gel packs to ensure sample is kept cool in transit.
- International clinics only: a copy of the FAVN REPORT FORM should be placed on the outside of the package for custom agents (which they may retain), along with an original FAVN form inside the package with the sample.
Use of an overnight or next day carrier is highly recommended. Shipping charges are the responsibility of the shipper - we do not accept packages sent "bill to recipient."

Samples may be stored up to one week in refrigeration before shipping. It is preferable to hold onto samples over the weekend. If it is urgent to send a sample for Saturday delivery, please contact us for alternative address.
The sample receipt date is assigned when packages are opened on Monday through Friday.
Sendout of FAVN REPORT FORM
The official FAVN REPORT FORM (stickered) is sent via regular US Postal Service mail to the submitting clinic. Copies of the official report are automatically emailed or faxed to the submitting clinic. Pet owners must obtain the results from the veterinarian.
To release the FAVN REPORT FORM to the pet owner or a location other than the submitting clinic we require:
- Written permission from the submitting clinic sent with the sample or faxed to us before the test is finalized
- The mailing address, emails and/or fax numbers for where to send the FAVN REPORT FORM
Clinics located outside the United States are sent the results via airmail.
To receive the FAVN REPORT FORM more quickly, please submit a "Courier Delivery Form" with the sample. This is highly recommended for clinics located in countries where the postal system is slow and/or unreliable.
The official FAVN REPORT FORM (stickered) is sent directly to Hawaii, Guam and islands in the Caribbean as required by those destinations. Therefore, the submitting clinic will receive a copy.
Copies of the FAVN REPORT FORM can be emailed or faxed to any government authority such as a USDA or destination official upon request.
Billing Policy, Charges, and Methods of Payment
The submitting clinic listed on the FAVN REPORT FORM is considered our client, not the pet owner. Please see our Payment Options & Billing Policy for further details. Prices are subject to change; please check the Cost Of Test for current prices.
The FAVN test can only be cancelled by written authorization from the submitting clinic/veterinarian faxed to the laboratory at 785-532-4522. Written authorization must be received prior to sample testing, otherwise charges still apply.
The original FAVN report with the results will be sent to the submitting clinic via the US Postal Service (USPS), and via regular USPS airmail for clinics located in countries other than the United States. There is no additional fee for receiving your results via regular mail USPS. However, it is not possible to track the envelope containing your results when they are sent by regular USPS mail, or to predict with precision when they will arrive.
To receive the original results more quickly you can request they be sent via courier delivery. This optional service is highly recommended for submitting clinics located in countries where regular mail can be slow and/or unpredictable. This service offers the additional benefit of allowing you to track your results in transit.
Laboratory hours are Monday-Friday, 8:00 am to 5:00 pm, excluding State Holidays.
The Compendium of Animal Rabies Prevention and Control and the Advisory Committee on Immunization Practices provide guidance that administration of rabies post-exposure prophylaxis is a medical urgency, not a medical emergency. Submissions received on Saturday will be tested the next working day. There is no routine testing on Sundays or holidays if the following day is a business day. The need for emergency testing will be evaluated on a case-by-case basis and arrangements MUST be made by telephone in advance.

The Courier Delivery Form provides two options for shipping out the results via courier. Please select only ONE of the following options.
Option 1: Payment made to my courier account.
- Enter the Animal’s Microchip Number(s).
- Check the courier service you have an account with.
- Enter your courier account number.
- Enter the contact information for this courier account.
- Enter an email for the recipient (if any).
Option 2: Payment of $50.00 made to my credit card. Results will be couriered to me via Federal Express (note: FedEx cannot delivery to a P.O. Box).
- Enter the Animal’s Microchip Number(s).
- Enter the credit card number and name, as on your credit card.
- Select the type of credit card.
- Enter the expiration date specified on your credit card as: month/year.
- Enter the CVV number (number on the back of your credit card).
- Enter your billing address and contact information.
- After filling out one of the above two options send this form with your FAVN sample(s), or if your samples have already been received, send this form via email or fax (785-532-4474).
NOTE: More than one result can be sent in the same courier shipment if the samples were received together. Please write all microchip numbers on the Courier Delivery Form. Each time overnight delivery is requested, this form must accompany the sample(s).
If you would like results returned via courier to a location other than the submitting clinic (for example directly to the pet’s owner), please attach with the Courier Delivery Form written permission from the submitting clinic to do so, including the new mailing address. If no written permission is received from the submitting clinic to send results elsewhere, the results will only be sent to the submitting clinic regardless of who pays for shipping.
